Lawyer Selection Terms:

• I am making this initial contact to The Morgan Law Group (MLG) by scheduling this appointment through MLG’s Setmore website: justice360.setmore.com. I will call you on the date and time scheduled at 833-833-2675. I intend to discuss the possible retention of MLG for my property damage claim to my insured property. While I may have received your contact information from Double G Construction, they provided me with the names of several law firms, and I made the decision on which firm I would interview concerning my claim.

Consent to Further Communication Terms:

• I hereby provide you with my consent and instruction to call me within 5 minutes of my scheduled appointment in the event I do not call you on time. If I do not answer at that time, you have my permission and consent to make additional, reasonable attempts to contact me, unless I instruct you otherwise in writing at intake@morganlawgroup.net. To ensure your inquiries are handled accurately, courteously, and promptly, phone calls among you and MLG or any of our affiliates, attorneys, agents, case managers, department heads, or paralegals may be recorded. We may from time to time make calls and/or send text messages to you at any telephone number associated with your account. The manner in which these calls or text (SMS) messages are made to you may include, but is not limited to, the use of prerecorded/artificial voice messages and/or an automatic telephone dialing system. You certify, warrant, and represent that the telephone numbers you have provided to us are your contact numbers. You represent that you are permitted to receive calls at each of the telephone numbers you have provided to us. You agree that MLG may send emails to you at any email address you provide us, or use other electronic means of communication to the extent permitted by law.
